The World Health Organization has updated its account of an Andes hantavirus outbreak linked to the cruise ship M/V Hondius, reporting a cross-border response that included medical evacuations, laboratory testing, passenger and crew repatriation, contact tracing, quarantine and monitoring.

As of May 27, WHO reported 13 cases and three deaths. Eleven cases were laboratory-confirmed and two were classified as probable. All of the cases in that update involved passengers or crew members aboard the ship.

The outbreak illustrates how an uncommon respiratory illness associated with maritime travel can create public-health responsibilities across national borders, particularly after passengers and crew members leave a vessel and return to different countries or territories.

How the outbreak developed

WHO said it was first notified on May 2, 2026, after severe respiratory-illness cases were reported aboard the M/V Hondius. The agency described the event as an Andes hantavirus outbreak associated with cruise ship travel.

The May 27 count formed the initial reported cluster: 13 cases, including three deaths. The distinction between confirmed and probable cases is significant. Eleven infections had been established through laboratory testing, while two met the classification for probable cases.

The cases were not limited to a single national public-health system. WHO’s later update referred to Canada, the Netherlands, Spain and Tristan da Cunha in connection with the broader response beyond the original ship cluster.

Case reported after disembarkation

In the later update, WHO reported a probable case in Tristan da Cunha involving a person who developed symptoms after disembarking from the cruise ship. The patient received treatment, recovered and was discharged.

That case underscores the practical difficulty of tracking possible exposure after a ship’s passengers and crew disperse. People may travel onward or return home before symptoms appear, requiring health authorities to identify contacts and determine who may need monitoring or quarantine.

WHO said the response included isolating cases and providing clinical management, as well as arranging medical evacuations where necessary. Authorities also carried out laboratory testing and worked on the repatriation of passengers and crew.

Why maritime travel matters

A cruise ship can connect people from multiple countries during a single voyage. When a serious respiratory illness is identified aboard the vessel, public-health officials must coordinate across jurisdictions rather than respond only where the ship is docked or where the first illness was detected.

The response described by WHO included international contact tracing, quarantine and monitoring. Those measures are intended to help authorities identify people who may have been exposed, watch for symptoms and limit further spread while cases are assessed.

WHO uses its Disease Outbreak News system for confirmed or potential acute public-health events that may have international travel or trade implications. The agency’s reporting framework therefore covers events in which travel itself creates a need for international coordination.

What is known now

The July 2 update does not state a newer cumulative case total beyond the 13 cases reported as of May 27. It does, however, document the probable case in Tristan da Cunha and the range of measures used in response to the outbreak.

The reported figures should not be read as a count of every passenger or crew member who may have been exposed. The May update specifically identified the 13 reported cases as passengers or crew members, with 11 laboratory-confirmed and two probable.

For health authorities, the immediate task described in the WHO updates is cross-border follow-up: testing, isolation and medical care for suspected or confirmed patients, along with tracing, quarantine and monitoring for people who may have been exposed during the voyage.
